  • Coordinates patient’s home care services, scheduling caregivers, coordinate plan of care and discharge planning
  • Monitors documentation and charting requirements that support services provided to meeting billing requirements.
  • Conducts daily assessments of patients and coordinates with other departments to satisfy patients’ needs and requirements
  • Conducts weekly case management meetings to review patient care status
  • Coordinates clinical operations to ensure compliance with Managed Care and Department of Health requirements.
  • Performs other tasks as assigned.

Qualifications

  • Chinese speaking required (Mandarin AND Cantonese)

Bilingual Chinese Customer Service

The essential function of the position within the organization is to provide support and assigning patient referrals, as directed, to agency para-professional staff under the direct supervision of the DPS/RN and ensuring the quality of customer service and care delivered by para-professional staff is up to company’s standards.

Daily Operations: Responsible for maintaining communication with case managers and agency personnel. Provides basic HR services; assisting with the hiring process and orientation, responsible for administrative supervision of paraprofessional staff, managing employee relation issues and processing complaints, providing support for benefits, serving as a departmental liaison, and performing related administrative functions, including creating, reviewing maintaining and submitting reports.

Responsibilities:

Qualifications:

  • Must befluent in reading, writing and speaking in Chinese Mandarin and English andCantonese.

  • One year experience of office work setting.
  • Good telephone, organizational, typing and communication skills necessary.
  • Ability to maintain accurate schedules, notes and records. ·
